I think it usually refers to a condition in the hospital that threatens the life of everyone inside. My dad wondered the same thing after last week's show, so he looked up it online, and apparently there's a whole message board discussion somewhere about it. Some guy said that it was a bomb threat, whereas others had the other definition. Funny thing was, my dad said "Maybe someone goes to the hospital with a bomb stuck inside him." My reply was "Yeah, right, I doubt that would happen," but go figure, he was right on! I'm starting to think he asked someone at work about it, either that or it was a super lucky guess.
